The Admissions & Discharge Officer will be responsible for both the admission and discharge process, providing patients and their families with clear information, support and guidance throughout their time at the hospital.


Requirements

  • A minimum of a Diploma in Nursing, Clinical Medicine & Community Health or related field
  • Three years of related work experience
  • Strong understanding of hospital admissions and discharge procedures.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and families from diverse backgrounds.
  • Proficient in computer skills and hospital information systems.
  • Ability to prioritize tasks, manage deadlines, and work effectively in a fast-paced environment.
  • Excellent organizational skills and attention to detail.
  • Compassionate and empathetic demeanor.

  • Schedule admissions for patients in accordance with physician recommendation and bed availability.
  • Collect and verify patient demographic and insurance information.
  • Explain hospital policies and procedures to patients and families, including billing, insurance coverage, and visiting hours.
  • Obtain informed consent for treatment procedures.
  • Coordinate pre-admission testing and ensure all necessary paperwork is completed.
  • Assist patients with getting settled in their rooms and connect them with appropriate nursing staff.
  • Work collaboratively with the care team to develop discharge plans that meet the individual needs of each patient.
  • Coordinate discharge activities, such as scheduling follow-up appointments, arranging transportation, and providing medication instructions.
  • Ensure all financial obligations are settled or payment arrangements are made.
  • Process paperwork and update patient records to reflect the discharge.
  • Provide emotional support and guidance to patients and families as they transition back home or to a new care setting.
  • Any other duties as may be assigned from time to time.
